



The South Haven School District will again provide school-time insurance coverage for all students. South Haven U.S.D. 509 has purchased these plans through Sports Management Associates, IC. Within limitations, this policy will incur medical expenses within one year from the date of injury up to a maximum amount for any one accident. Included will be a policy for athletic coverage. This policy provides much better benefits in the event of a disabling injury. Parents of all students need to be aware of the guidelines established by the insurance company. This policy acts as a secondary insurance policy which means claims must first be filed with the student’s own health insurance company for processing before turning in a claim to the school’s insurance carrier. Following this procedure, the insurance carried by the school will then review and determine the amount of benefits to be paid. Parents are advised that there are limitations and guidelines imposed by the insurance company.Transportation of participant students are covered only if it is directly to or from the covered event, authorized by the school district and is paid for or subject to reimbursement by the school. There is no longer coverage if transportation is provided by individuals or volunteers.Athletes/students receiving medical attention for an injury that occurred at a school event or during school hours are to report the injury to the coach or sponsor immediately. After the coach or sponsor is notified by the injured student, the correct forms can be completed by the school officials.
