Just a few reminders to parents in preparing for school enrollment.
New forms will need to be completed and signed by both a parent/guardian and physician for all asthma inhalers used by students in school activities and any prescription medication to be given on a regular basis at school. Forms will be given out at enrollment by the school nujrse and will need to be returned by the first day of school.
Reminder letters have been sent to the parents/guardians of all students needing immunizations before attending school again. Documentation of those immunizations may be brought to enrollment and must be turned in to the school nurse by the first day of school.
New forms will need to be completed and signed by parents/guardians giving permission to the school nurse to administer various over-the-counter medications at school. Those forms will also be provided at enrollment. No over-the-counter medications will be given to any student without a signed consent by a parent or guardian.
Any student attending the school for the first time under the age of nine will need to have a state-required health assessment done by a health care provider. Forms will be provided at enrollment.
Parents/guardians will be asked to complete a new health update form for each student.
