ELIGIBILITY REQUIREMENTS FOR SPORTS

In addition to those scholastic requirements as set forth by the Kansas State High School Activities Association, any student of the South Haven School District involved in extracurricular activities must be passing in six subjects of unit weight on a week-to-week basis in order to be scholastically eligible to participate in that event. Eligibility for the week will be determined the previous Friday.

Student Insurance

All athletes 6-12 will be insured against accidents and injuries by a policy purchased by the Board of Education through Student Assurance Services of Stillwater, Minnesota.  This is a LIMITED POLICY WHICH COVERS A PORTION OF THE EXPENSE NOT COVERED BY THE FAMILY INSURANCE.  It is a responsibility of the student and/or parent to inform the office when they need to file a claim.  Claims MUST be made within 90 days.

The KSHSAA Catastrophic Insurance which covers major athletic injuries (7-12th grade) has also been purchased by the Board of Education.

Student Assurance Services also offer a voluntary policy for K-12 students.  Details of this policy will be sent home with students and information is available here.

STUDENT PHYSICALS

All students in grades 6-12 who plan to participate in any sport or cheerleading must have their physical forms completed and signed by a healthcare provider with the credentials of MD, DO, DC, PA-C, or APRN AND a parent or guardian on or before the first day of practice in order to participate.  In addition to the physical form, students and parents must complete a KSHSAA Recommended Concussion & Head Injury Information Release Form.  Students may pick up these forms in the school office if they desire to use their personal family physician or clinic.  Copies or other forms will not be accepted.  These forms can also be found at right  on the Athletics page. Only approved forms will be accepted for participation.  Athletes must have a completed current physical on file with the athletic director before the athlete can practice in the sport of choice.
